Rehabilitation Pathways for Different Needs

Home rehabilitation can reduce travel burden and support function in the patient’s daily environment, but it is not suitable for every person or every stage of a condition.

Pilot categories

What the Pilot Will Support as Capacity Develops

Post-operative recovery

Recovery after orthopaedic surgery and support after discharge.

Joint replacement

Structured mobility and function pathways after joint replacement.

Neurological rehabilitation

Home support following stroke and for selected neurological needs.

Older-adult mobility

Balance, mobility, confidence, and fall-risk support for older adults.

Musculoskeletal limitations

Pain, stiffness, weakness, and movement limitations where home care is suitable.

Sports rehabilitation

Progressive rehabilitation and return-to-activity support after sports injuries.

Pediatric rehabilitation

Selected child and family-centred rehabilitation pathways.

Long-term mobility support

Support for disability, long-term mobility needs, and functional independence.

Functional exercise

Individualized exercise and return-to-activity programmes.

Important limits

Home Is Not Always the Right Setting for Care

Availability depends on pilot location, clinician competencies, patient suitability, and service capacity. Some patients may require clinic, hospital, medical, or multidisciplinary care instead of—or alongside—home physiotherapy.

  • Identity, qualification, and licence verification
  • Competency review and case-suitability rules
  • Clear clinical boundaries and consent
  • Minimum necessary data collection
  • Structured documentation and audits
  • Red-flag escalation and incident review
  • Complaints, replacement, and performance processes
  • Secure information handling
Emergency notice

Physiawy is not an emergency service. Chest pain, severe breathing difficulty, sudden weakness, loss of consciousness, severe trauma, or other urgent symptoms require immediate emergency or hospital care.
