Physiawy is not designed as a list of professionals. The model connects matching, assessment, treatment, documentation, and review in one pathway understood by patients, families, and professionals.

How the Pathway Works, Step by Step

    Request Care

    The patient or family shares the general rehabilitation need, area, availability, preferences, and whether a physician referral exists.

  2. 02

    Review and Matching

    Physiawy reviews general home-care suitability, location, required competencies and professional grade, availability, and patient preferences.

  3. 03

    Specialist or Consultant Acceptance

    Responsible Physiotherapy Specialist or Consultant

    An eligible Physiotherapy Specialist or Consultant accepts responsibility, confirms the assessment arrangement, and defines the required supervision level.

  4. 04

    Assessment and Treatment Plan

    Responsible Physiotherapy Specialist or Consultant

    The responsible Specialist or Consultant assesses the patient, confirms home-care suitability, establishes goals, approves the treatment plan, and defines review points.

  5. 05

    Treatment Delivery

    Specialist, Consultant, or supervised Practitioner

    Sessions are delivered directly by the Specialist or Consultant, or by a verified Physiotherapy Practitioner working under active supervision.

  6. 06

    Documentation and Supervision

    Care team with identified supervision

    Every session records attendance, treatment, response, progress, concerns, barriers, escalation needs, and the next step. Practitioner records remain available to the assigned supervisor.

  7. 07

    Progress Review and Reassessment

    Responsible Physiotherapy Specialist or Consultant

    The responsible Specialist or Consultant reviews documentation, progress, response, adherence, concerns, and whether reassessment or treatment-plan changes are needed.

  8. 08

    Next Clinical Decision

    Responsible Physiotherapy Specialist or Consultant

    The responsible Specialist or Consultant may approve continuation, modification, reassessment, physician review, escalation, self-management, or discharge.

The professional delivering the sessions is not always the same person responsible for assessment and clinical supervision. Physiawy makes both responsibilities clear.

Physiotherapy Specialist or Consultant

Assessment and Clinical Responsibility

A licensed Physiotherapy Specialist or Consultant assesses the patient where appropriate, determines whether home physiotherapy is suitable, establishes rehabilitation goals, approves the treatment plan, defines the required level of supervision, and reviews progress.

Physiotherapy Practitioner

Treatment Under Supervision

A licensed Physiotherapy Practitioner may deliver the approved home sessions under the supervision of the assigned Specialist or Consultant. The Practitioner follows the treatment plan, documents every session, and reports progress, barriers, or concerns.

Direct care

Direct Specialist or Consultant Care

Where appropriate, the assigned Specialist or Consultant may conduct both the assessment and the ongoing treatment sessions directly. A Practitioner is not required in every patient pathway.

At the initial assessment, the responsible Specialist or Consultant defines the documentation-review frequency, direct-supervision requirements, formal reassessment points, escalation rules, and whether continuation beyond a milestone requires approval.

A review around session 6, 12, or 24 may be used where appropriate, but the same schedule does not apply to every patient or condition.
